boembolic events, seven were TIAs without major sequelae, two patients had permanent deficit and one patient died. Eighty-eight patients remained on long-term anticoagula-tion because of AF. There were six reported haemorrhagic complications, for an actuarial rate of 95.6"1.97% patients free of anticoagulation related haemorrhage at 15 years for the total series and 91% for the 88 patients receiving anticoagulation. 7. Clinical bottom line The current European Society of Cardiology guidelines support the use of warfarin for 3 months post-mitral repair, citing an absence of studies supporting the safety of omitting warfarin. They acknowledge that this is based on expert consensus and that many surgeons do not follow this guideline. The longest follow-up studies of patients post-mitral repair report excellent results using short term warfarin, and they also show that a third of patients discharged in sinus rhythm will have an episode of atrial fibrillation shortly after. In addition, the highest risk of thromboembolism occurs in the early months post surgery. Therefore, until studies demonstrate the safety of omitting warfarin for patients undergoing mitral valve repair 3 months of anticoagulation should remain the standard of care. for the management of patients with valvular heart disease. Retrograde cerebral perfusion and delayed hyperbaric oxygen for massive air embolism during cardiac surgery.
We report a case of massive air embolism from a ventricular vent line during cardiac surgery successfully treated with emergent retrograde cerebral perfusion and delayed hyperbaric oxygen therapy. The etiologies of this rare but potentially devastating complication are discussed along with prevention and treatment options.
متن کاملEfficacy of SOFT COAG for intraoperative bleeding in thoracic surgery.
We present and examine two cases of the dramatic hemostasis with SOFT COAG in general thoracic surgery. SOFT COAG is a coagulation mode unique to VIO electrosurgical units (ERBE Elektromedizin GmbH, Germany). This system regulates the temperature rise below boiling point without generating sparks, which is high enough to denature protein. In addition to clinical applications, this coagulation s...
متن کاملGiant pseudoaneurysm on left ventricular posterolateral wall with an orifice between papillary muscles.
A left ventricular pseudoaneurysm develops when myocardial rupture is contained by the pericardium. Although left ventricular pseudoaneurysm has been a topic of discussion since the pioneering days of open heart surgery, it still remains a technical challenge in reconstructive cardiac surgery. Reoperation following pseudoaneurysm repair is also frequent. We report surgical treatment in two pati...
متن کاملUncovered stent-graft in the treatment for residual patent false lumen after surgical repair for acute type A aortic dissection.
Patent false lumen after surgery for acute type A aortic dissection (AAAD) influences the outcome with increased risk of aortic rupture and visceral malperfusion. We report a case of a patient previously operated on for aortic valve repair and replacement of the ascending aorta because of AAAD, presenting at follow-up with a retrograde dissection in aortic arch, persistent patent false lumen an...
متن کاملHeart team discussion in managing patients with coronary artery disease: outcome and reproducibility.
Recent ESC/EACTS revascularization guidelines advocate a 'Heart Team' (HT) approach in the decision-making process when managing patients with coronary disease. We prospectively assessed HT decision-making in 150 patients analysing personnel attendance, data presented, the 'actioning' of the HT decision and, if not completed, then the reasons why. Additionally, 50 patients were specifically re-...
متن کاملImpact Factor 1.112 for Interactive CardioVascular and Thoracic Surgery: a well-deserved recognition for all contributors.
Interactive CardioVascular and Thoracic Surgery (ICVTS: www. ICVTS.org) has been allocated a 2012 Impact Factor (IF) of 1.112. This is a very adequate reward for all contributors who have entrusted their work to ICVTS, the editorial team, the sponsoring European Association for Cardio-Thoracic Surgery (EACTS, www. eacts.org) and the publishers. It is worthy of note that ICVTS was founded as a s...
